Description of original award (Fiscal Year 2008, $223,587)

KidsPeace has developed KidConnect, a program that will provide coordinated case management for aftercare services to West Virginia youth starting at the time of their admission into an out-of-state KidsPeace facility. Direct services to youth and families, including coordination with in-state resources, will extend six months post-discharge. In addition, KidsPeace will provide open-ended aftercare services to any youth throughout West Virginia through the use of its innovative online counseling website, TeenCentral.net. Progress will be measured through youth and family participation in all aspects of the program. NCA/NCF
