Description of original award (Fiscal Year 2007, $98,723)

Bloomfield Board of Education is contracting with the Bloomfield Connections program. The Bloomfield Connections is a pilot program that specializes in case management services to students in bridging the transition between middle and high school. These services are designed specifically for the incoming freshmen students who have been identified by the Bloomfield Middle School SWAS (school within a school program). These students have multiple behavioral problems related to academic issues and truancy and includes students who have not met the criteria for special education. Bloomfield Connections will address those needs and assist them to overcome these barriers thus reducing their chances of entering into the juvenile justice system. The components are: a) community work; b) case management; c) parent group program; and d) enrichment programs. These students will be assigned a case manager who will assist them and their families in developing an individual action plan. This plan will prescribe specific actions to be taken by the student to address those needs, i.e. tutoring after school, counseling individual and/or family, treatment substance abuse, anger management and community services. At the end of the school year a comparison report comparing a control group and the Bloomfield group using the high school's behavior incident reports, attendance and academic records and input from the case manager and action plan will be included in this evaluation report.
