Description of original award (Fiscal Year 2007, $974,917)

Erie County proposes an expansion of an already existing Wraparound initiative established within the county to assure continuity and consistency in the system of care for youth and their families. The main goal of this project is to utilize and expand the Wraparound process to empower families and build on their capacities to nurture their children and raise them successfully. The project will be focused on multi-need, at risk youth, and youth reentering the community from the Ohio Department of Youth Services (DYS) as its target population. These children are in need of many systems for support. They are involved in multiple systems; and as such remain at risk for institutionalization; present as a systems' failure; and experience multiple failures across multiple systems. The objectives of this Wraparound project include the following: increase family-centered services that address the needs of the family as a whole; enhance partnerships across service systems such as health, education and workforce development; community and faith based organizations; and business and employers; strengthen the capacity of the family to function self sufficiently, including prevention of teen pregnancies; and engaging families in decisions, goal setting, and problem solving. A comprehensive evaluation plan has been designed to ensure optimal function of the project and that findings and outcomes are reported to OJJDP accordingly. CA/NCF
