This project aims to improve the identification of, and tracking service delivery to, children of persons incarcerated in the Philadelphia Prison System. Policies and systems for recording the children of inmates will be revised and upgraded eliminating incomplete or duplicated information and making this information available to social services with the permission of the inmates. At least 1000 children will be enrolled in the program to receive services and have their service consumption tracked. Utilizing Food for Life's comprehensive Client System software, the grantee will establish and implement an online client tracking and case management database application specifically tailored for the needs of beneficiary children, service providers and correctional agencies. The grantee will provide coordination of counseling, training and social services to evaluate prior conditions such as physical and/or mental abuse, abandonment issues, spousal and child support, etc. and prepare both the parent and the children for the rebuilding process where appropriate. The grantee will monitor scholastic performance of the children and provide them with intervention programs and services that generate support transition of offender and child towards a productive life. The grantee will verify and coordinate medical and dental treatment and nutritional support for participating children.
