Providence Holy Cross Medical Center in San Fernando Valley, CA provides specialized services to individuals that may face employment barriers and/or safety issues as the result of past decisions. The Tattoo Removal Program offers a unique service for youth and young adults to remove visible tattoos that represent a former lifestyle they are trying to change. The goal of the Tattoo Removal Program is to empower at-risk youth and young adults to find employment, decrease their risk of injury on the street, and increase their ability to change lifestyles and become self-sufficient. Providence Holy Cross Medical Center's regular Improving Organizational Performance (IOP) evaluation process will rate the performance of the Tattoo Removal Program. An internal Program Evaluator will track outcomes through client focus groups, client tracking surveys, and an exit questionnaires given to clients when they complete their treatments. Expected outcomes include: increase in participant self-esteem, job success and GED achievement. The grantee will report their progress in their Categorical Assistance Progress Reports.NCA/NCF
