Description of original award (Fiscal Year 2003, $248,375)

The Interagency Staffing Team (IST) wraparound service plan addresses the need for holistic prevention and intervention services that serve youth and their families with multiple needs, including those that are not met by extant programming. The multidisciplinary team assesses referred youth (ages 8 through 13) and their families using a strengths-based model. With the youth and family, the team devises an individualized Family Service Plan.
An assigned case manager coordinates the team's services to the family, and an intensive case monitor is assigned to mentor the youth when appropriate. The youth and family receive services that are provided for by the existing social service structure (in a coordinated, non-duplicative effort) as well as non-traditional services such as respite care for parents, school field trips for youth, etc. This one-year proposal will fund the development and implementation of IST's in three critical areas of Lee County, and will grow to serve approximately 30-40 youth concurrently.

The primary goal of the IST program is to provide unconditional care in a culturally competent manner to at-risk youth aged 8-13 living in identified high-risk neighborhoods and their families so that these youth have a reduced risk of becoming serious, chronic, or violent delinquents. IST program activities will: (1) develop a confidential referral system where an agency, parent or concerned party can refer a youth for individualized staffing; (2) develop a method for pooling multi-agency resources that would knock down the barriers between the providers and establish new ways to cross boundaries for the purpose of development and implementation of ONE holistic and multi-disciplinary family treatment plan (FTP) for each at-risk youth and family; and (3) develop a multi-agency case management method where one agency takes the lead in the FTP implementation and reports back to the Team on the progress.

Anticipated outcomes for the IST include: (1) reduction in youth delinquency; (2) improved family functioning; (3) improved school attendance; (4) improved coordination of agencies' involvement with family; (5) reduction of overlap and duplication of services; and (6) reduction in disciplinary referrals (in school).
