100 Black Men of San Antonio, Inc. will support direct transitional services to help moderate- to high-risk 15- to 18-year-old youth to successfully reintegrate into their communities. This will be accomplished by planning and implementing a problem-solving, specialized job skills reentry exposure model for juvenile justice involved youth transitioning from a residential or correctional treatment facility back into community. The program model engages and assists youth and families from the justice system using the Transition from Jail to Community Initiative (TJC Model).
