Community Care & Connect in Nelson County addresses school violence by providing intensive case management and integrated services to students enrolled in the New Horizons Academy. Primary Project Activities include:
1)Develop and annually update a school safety strategic plan to address mental health crises and concerns;
2)Integrate services with CBOs that have experience in operating violence prevention and early identification programs with youth and families;
3)Implement strategies that provide youth with individualized service backpacks and engage family as a critical change agent; and
4)Implement prevention programs for all students to supplement targeted interventions and build a positive school climate.
Primary beneficiaries are youth in grades 6-12 in Nelson County, Kentucky who demonstrate need for intensive treatment and restorative care based upon substantiated risk for or victimization related to violence and/or abuse.
The anticipated outcomes include:
1) Integrated services through partnerships with community-based organizations to deliver unified and intense supports to targeted students.
2) Implementation of a case management approach with ongoing communication and involvement of families.
3) Direct services and interventions with in the school setting to address root causes of threats of harm to self or others.
4) All students and staff are provided with violence prevention and recognition strategies to build a positive school climate using a truama informed approach.
